FOR FAST AND ACCURATE SERVICE

1. If you are requesting a background check for employment or licensing purposes you must provide the agency's name and authorization number.

2. If the background check is being sent to a government agency, a Z - ORI number may also be required.

3. You must bring a valid form of government issued photo identification.

4. The form below should be completed prior to your appointment with Abruzza.

5. Payment is due at time of appointment, we accept major credit cards and debit cards, exact cash, PayPal, Zelle or Apple Cash.
